Terms of Service

Welcome to Quality Paramedical Services. By accessing our website or using our services, you agree to the following Terms of Service (“Terms”). Please read them carefully before scheduling or receiving services.
1. Acceptance of Terms
By using this site or our mobile phlebotomy services, you confirm that you understand and accept these Terms, as well as our Privacy Policy and Security Policy. If you do not agree with any part, please do not use our services.
2. Services Provided
We provide mobile phlebotomy and specimen collection services at the client’s location (home, workplace, etc.). These services may be part of:
Physician-ordered diagnostic testing
Employer-sponsored wellness events
Specialty lab programs
We reserve the right to modify or discontinue services at any time.
3. Eligibility
To use our services, you must:
Be at least 18 years old or have consent from a parent or legal guardian
Provide accurate, up-to-date personal and medical information
Have a valid lab requisition or be participating in an approved testing program
4. Appointments and Cancellations
Appointments must be scheduled in advance. All appointments must be confirmed the day before, either by phone call or text message. Unconfirmed appointments may be subject to cancellation.
If you need to cancel or reschedule:
Notify us at least 24 hours in advance to avoid charges
Cancellations made within 24 hours or missed appointments may be subject to a service fee
We reserve the right to cancel or reschedule appointments due to safety concerns, weather conditions, or technician availability
5. Payment Terms
Payment is required at the time of service. We accept all major credit cards and other pre-approved payment methods. Mileage fees are set at the standard IRS rate .70 cents per mile after 25 miles.
6. Refund Policy
Due to the nature of our services, all sales are final once the service has been rendered.
Refunds may be issued only under the following conditions:
Prepaid appointment canceled at least 24 hours in advance
A billing or scheduling error occurred on our part
The service could not be completed due to a Quality Paramedical Services issue (e.g., technician no-show)
If a specimen requires a redraw due to an error on our part, we will perform the redraw free of charge at the earliest convenience.
To request a refund or report a service issue, please contact us at contact@qualityparamedicalservices.com within 7 days of your original appointment. Refunds, when approved, will be returned to the original payment method.
We do not issue refunds for missed appointments, late cancellations, or dissatisfaction with results from third-party laboratories.
7. Specimen Handling and Results
All specimens are collected and sent out on the same day of your appointment. Specimens are typically delivered to the designated laboratory within 1 business day, depending on location and courier schedules.
Please note:
Quality Paramedical Services is not responsible for delays caused by third-party couriers or laboratories.
The turnaround time for test results is determined by the laboratory or your physician.
For information on when you can expect your results, you must consult directly with the ordering physician or testing laboratory.
We do not interpret lab results and do not provide diagnostic or medical advice.
8. Confidentiality and HIPAA Compliance
We comply with all HIPAA regulations and ensure the confidentiality of your medical and personal information. Your data will only be shared with authorized parties such as:
Laboratories processing your specimens
Your healthcare provider (if required)
Wellness program administrators (if applicable)

9. User Conduct
By using our services, you agree not to:
Use false information or impersonate another person
Disrupt or interfere with the delivery of our services
Attempt to access unauthorized systems or data
Violation of these terms may result in termination of service without refund.
10. Intellectual Property
All content on this website, including logos, branding, text, and images, is the property of Quality Paramedical Services and may not be used without written permission.
11. Disclaimer of Warranties
Services are provided "as is" and "as available." We do not guarantee specific outcomes, lab results, or uninterrupted availability of services. Quality Paramedical Services disclaims all warranties to the maximum extent allowed by law.
12. Limitation of Liability
We are not liable for any indirect, incidental, or consequential damages, including issues resulting from third-party laboratories, missed appointments, or misuse of services.
13. Governing Law
These Terms are governed by the laws of the State of New York, without regard to its conflict of law rules.
14. Modifications to Terms
We may update these Terms at any time. Updated versions will be posted on this page with a revised effective date. Continued use of our services constitutes your acceptance of any changes.
