1. Acceptance of Terms
By using this site or our mobile phlebotomy services, you confirm that you understand and accept these Terms, as well as our Privacy Policy and Security Policy.
2. Services Provided
QPS provides mobile phlebotomy and specimen collection services at the client's location, including home and workplace visits. Services may be part of physician-ordered diagnostic testing, employer-sponsored wellness events, or specialty lab programs.
3. Eligibility
To use services, you must be at least 18 years old or have consent from a parent or legal guardian, provide accurate personal and medical information, and have a valid lab requisition or approved testing program.
4. Appointments and Cancellations
Appointments must be scheduled in advance and confirmed the day before by phone call or text. Notify QPS at least 24 hours in advance to cancel or reschedule. Late cancellations or missed appointments may be subject to a service fee.
5. Payment Terms
Payment is required at the time of service. QPS accepts major credit cards and other pre-approved payment methods. Mileage fees are set at the standard IRS rate of 70 cents per mile after 25 miles.
6. Refund Policy
All sales are final once service has been rendered. Refunds may be issued for qualifying prepaid cancellations, billing or scheduling errors, or when service could not be completed due to a QPS issue. Redraws caused by an error on the part of QPS will be performed free of charge at the earliest convenience.
7. Specimen Handling and Results
Specimens are collected and sent out on the same day of appointment when possible and are typically delivered to the designated laboratory within 1 business day depending on location and courier schedules. QPS is not responsible for third-party courier or laboratory delays and does not interpret lab results or provide diagnostic or medical advice.
8. Confidentiality and HIPAA Compliance
QPS complies with HIPAA regulations and protects medical and personal information. Data is only shared with authorized parties such as laboratories, healthcare providers, or wellness program administrators when applicable.
9. User Conduct
Users agree not to provide false information, impersonate another person, disrupt services, or attempt unauthorized access to systems or data.
10. Intellectual Property
Website content, logos, branding, text, and images are property of Quality Paramedical Services and may not be used without written permission.
11. Disclaimer of Warranties
Services are provided as is and as available. QPS does not guarantee specific outcomes, lab results, or uninterrupted service availability.
12. Limitation of Liability
QPS is not liable for indirect, incidental, or consequential damages, including issues resulting from third-party laboratories, missed appointments, or misuse of services.
13. Governing Law
These Terms are governed by the laws of the State of New York.
14. Modifications to Terms
QPS may update these Terms at any time. Updated versions will be posted on this page with a revised effective date.
